Domestic Adoption Application
How To Apply: Five
Easy Steps
1. Please read
through the material on the Alliance for Children
web site before applying, including our Domestic
Adoption Program Overview and Domestic
Adoption Qualifications.
2. Open and
print the following domestic adoption application
(Adobe
Acrobat Reader software is needed to view
and print the PDF file). Please note that this
application is for domestic adoption only. See
our international adoption
section if you wish to fill out an application
for international adoption.

3. Complete the
domestic adoption application and enclose with:
- two recent photos of each applicant
- a non-refundable application fee of $250
4. Mail completed
domestic adoption application to:
Alliance for Children, Inc.

5. After receiving
your adoption application, we will send you a confirmation
letter and a full application package.
